Project Description:
A Health and Social Care Worker dedicated to West Middlesbrough works as part of a five-strong team established in March 2003 to make sure national standards in the care of the elderly were met in Middlesbrough.
They work with residents over 75 who are not already accessing mainstream services. The team encourages pro-active prevention measures in the care of older people, meaning that they are less likely to need hospital treatment, and experience better general health.
In the first two years of the service over 3000 people have been visited in their own homes across Middlesbrough.
