Improving health and care

 

 

The Health, Opportunities and Work programme continues to bring together a number of initiatives to help improve the health and well being of West Middlesbrough residents, with WMNT working in partnership with Middlesbrough Primary Care Trust (PCT).

 

Key achievements 2004 - 2005

  • A £215,000 benefits windfall for local residents thanks to a new Welfare Rights Officer for West Middlesbrough, who supported 141 residents with information and advice.
  • 100 people took part in a range of community based health and lifestyle activities developed by WMNT’s health team. Key projects including a community health development worker were mainstreamed and are now core services provided by Middlesbrough PCT.
  • A Health and Social Care Worker for the over 75s worked with 154 older residents to help them take pro-active, preventative measures to take care of themselves and reduce the need for hospital treatment.
  • A Disability Forum was set up as a way for disabled residents to have a say on issues that affect them.
  • The Health Through Warmth initiative helped 24 people whose health was at risk because of cold and damp homes to improve their living conditions and the energy efficiency of their homes.
